Medical Referrals

Forster Private Hospital’s medical and surgical ward works in partnership with Physicians and General Practitioners, providing the best possible environment for your patients requiring inpatient care for their medical condition.

If you think your patient may benefit from a medical admission under one of our qualified specialists, allow us to help you by following the admissions process outlined below.

1. Does your patient need medical admission to hospital?
2. Does your patient have Private Health Insurance, DVA Status, Work Cover or the ability to self-fund their admission?
3. Call our Medical Admissions Team for assessment on 02 6555 1333
4. Fax completed referral form for health fund check to 02 6555 8750
5. Our Medical Admissions Team will arrange an Admitting Consultant and confirm patient’s admission time within one hour

Inclusion Criteria

• Blood Transfusions
• Cellulitis
• Chest Infections / Pneumonia
• Chronic Obstructive Pulmonary Disease
• Endocrinology
• Falls / Balance Management
• Gastroenterological Conditions
• Heart Failure
• Iron Deficiency Anaemia
• Ongoing Post Operative Care
• Pain Management
• Palliative / End of Life Care
• Parkinson’s Disease
• Urinary Tract Infections
• Wound Care

Exclusion Criteria

• Patients who require ICU/CCU/HDU or Emergency Department admission
• Falls with head strike (acute)
• Acute Stroke
• Acute Heart Attack

Admissions to Forster Private Hospital must be under the care of one of our treating Medical Practitioners. If you need assistance with selecting a Medical Practitioner, our Admissions and Assessment team can assist you.

Rehabilitation Referrals

Forster Private Hospital’s Rehabilitation Ward and Day Rehabilitation Program works in partnership with Physicians and General Practitioners to provide a comprehensive rehabilitation service.

If you think your patient may benefit from our rehabilitation service under one of our qualified specialists, allow us to help you by following the admissions process outlined below.

1. Does your patient have a medical reason to be admitted to hospital for their rehabilitation program? If not, your patient may be a candidate for our Day Rehabilitation program
2. Does your patient have Private Health Insurance, DVA Status, Work Cover or the ability to self-fund their admission?
3. Call our Rehabilitation Admissions Team for assessment.

• Neurological (Stroke, Parkinson‘s Disease, MS)
• Pain Management (Back, neck pain and arthritis)
• Orthopaedic including:
  • Post Surgery
  • Post Joint Replacement
  • Fractures
  • Post Surgical Revision
  • Acute Arthritis
• Amputees
• Trauma

Exclusion Criteria

• Comprehensive burns
• Acute Spinal Cord Injury
• Acute Traumatic Brain Injury with behaviour disorder
