If you would like to refer a private or public medical patient to the Hospital, and you do not have admitting rights, we have a list of Accredited Physicians at our hospital who could take your admission.

Alternatively you may wish to contact the In Charge Nurse at the Hospital on 02 6555 1317 for assistance in finding an appropriate admitting GP.

Procedure for Out of Hours Admission of Community Palliative Care Patients

Palliative Care patients can now be admitted after hours via the following process:

  • Patient’s condition noted to be deteriorating or carer noted to be having increasing difficulty managing care at home
  • Out of hours admission form sent to GP for Completion. Palliative Care to advise on form if known ACD or ambulance care plan in place
  • GP to complete Out of Hours admission form, an up-to-date Patient Summary and a copy of an ACD if the GP has one. This is to be faxed to Community Palliative Care on 02 6539 6364.
  • The Community Palliative Care nurse will scan this information into CHIME and note on the patient Information form that Hospital admission has been set up.
  • A copy of the Admission form/ documents to be given to the patient Carer
  • The original documents are to be placed in a plastic sleeve in the Out of Hours admission folder at Forster Private Hospital.
  • At weekly MDT meetings the Folder should be reviewed, updated information can be provided on patients and files of deceased patients removed.
